Position Overview
The Rehabilitation Coordinator provides direct services and support to adults with severe mental illness in a licensed, Medi-Cal–certified outpatient clinic. This position assists with client programming, supervision, crisis intervention, documentation, and skill-building activities under the guidance of the Program Manager.
Key Responsibilities
- Provide on-site monitoring, supervision, and care to mentally ill clients.
- Assist with development, implementation, and documentation of individualized service plans.
- Help implement daily schedules, programs, and rehabilitation activities.
- Observe, interact with, and respond appropriately to client concerns and behavioral needs.
- Provide emotional support, encouragement, and positive role-modeling.
- Deliver crisis intervention and perform emergency procedures as needed.
- Organize, assist, and supervise daily group rehabilitation services.
- Provide modeling and instruction in activities of daily living.
- Complete daily client progress notes accurately and on time.
- Meet monthly individual productivity expectations.
- Attend staff meetings, required training, and certification updates.
- Report unusual incidents or concerns to supervisor immediately.
Required Skills & Abilities
- Basic math and English language skills.
- Basic Microsoft Office skills, Outlook and computer work.
- Must be able to pass a background & live scan check.
- Valid CA driver's license, reliable transportation, clean DMV record.
- Current First Aid and QBS certifications.
Education & Experience
- Bachelor's degree or Associate Degree or equivalent combination of education and experience.
- 2 years related experience proving service to mentally ill or other disabled population; OR a combination of education and experience.
Physical Requirements
- Regular sitting, walking, reaching.
- Must maintain physical and mental health sufficiently to perform duties.
